Expectations
First Time Visit
On your first physical therapy visit, there are a few things to be aware of:
INTAKE FORM
You may fill out our intake form and bring it with you to your first visit, or fill it in at the office.
PRIVACY
This is our privacy policy notice.
SCRIPT
Most insurances require a script from your physician for physical therapy. This can obtained from your general practitioner, chiropractor, nurse practitioner, or specialist (i.e. an orthopedic doctor).
INSURANCE
Please check your benefits for outpatient PT. Our office can check your these, but we often can’t access all your information and therefore can’t guarantee billing amounts. Please bring the insurance card and ID with you.
TIME
Your first treatment will last around an hour. Subsequent treatments will last around 45-60 minutes. We can always accommodate time constraints, but please try to allow adequate time to make the most of it!
ATTIRE
Your first visit usually includes your evaluation and first treatment, meaning you should dress in a way that allows you to move comfortably! Please avoid skirts and high heeled shoes if possible.
INSURANCE & BILLING
ACCEPTED INSURANCES, IN-NETWORK VS OUT-OF-NETWORK, AND PAYMENT OPTIONS
We currently accept Blue Cross, United Health, Cigna, Medicare, and Neighborhood Health of RI. Some out-of-state plans may not be in our network. As a result, these plans may not be covered or may require a higher co-pay.
Please call your insurance and ask specifically if your plan is in Network with Form & Function Physical Therapy for any out of state BX/UH plans. We collect co-pays and estimated co-insurances or deductibles at time of treatment.
If you carry an insurance plan we do not accept we do have other payment options. Many people will have a Flexed Spending Account (FSA) or Health Savings Account (HSA). With a script from your physician use can pay using your FSA or HSA. In the rare instances that this charge is questioned, we will provide you with a statement of charges that you can submit with your script to your provider to validate the charges.
Our self-pay rates are $80 for the initial visit, and $65 for subsequent visits. We do also offer payment plans to ease the burden of these costs.
